SHARE respects your right to privacy. All staff (including volunteers & practicum students) of SHARE sign an oath of confidentiality when they are hired. SHARE will seek personal information from you to understand your circumstances and to help you and your family and all personal information is held in confidence. SHARE will only ask for information that is relevant to the work we do with you. You may at any time decide to not provide the information. If you decline to give us any or all of the information asked for, we will ensure that you understand how your choice may or may not impact our work together.
Clinical information may be shared verbally with other members of your child’s treatment team at SHARE and with other service providers involved with your family and child (with your consent).
We want you to be aware, however, that there are some ways we may be required to share information about your child:
- Statistical information is reported to the funder, the Ministry for Children and Family Development; and
- We are legally (and morally) required to report if there is a suspicion or concern that a child is at risk of, or is being abused or neglected; and
- We are legally required to report circumstances where not doing so would result in a clear danger to others.
Information Sharing
Sometimes it is useful to share information or reports from other professionals your child may be seeing or has seen in the past. In order to allow us to do this, you will be asked to complete a“Consent to Share Information” form. Any information received will not be passed on to other services without your consent. By signing this form you are giving SHARE staff permission to share information and reports with specified agencies or individuals that may help in developing a speech/language therapy, occupational therapy and/or physiotherapy program for your child or we may be referring your child for other services outside SHARE and you have consented to the release of information to facilitate service planning.
Distribution of Reports to Agencies Concerned With Your Child
You will receive a copy of all reports we write about your child. If you have any concerns or questions about reports you receive, please contact your child’s therapist. In the event that you want us to distribute the report to another agency or individual, we will need prior written consent from you.
Consent to Audio/Videotape Sessions
We may use photographs, audio/video recordings, or live monitoring of sessions for the following purposes:
- As part of ongoing therapy with your child; and
- For supervision, education and training purposes.
- Photo may be used for promotional purposes (with your written consent)
We will ask you to sign a consent form to give us permission to make audio and videotapes. All audio and video tapes will be erased (or otherwise destroyed) when your child is discharged.
